THYROÏDECTOMIE TRANS-ORALE ENDOSCOPIQUE PAR VOIE VESTIBULAIRE ANTÉRIEURE (TOETVA) : première expérience en France à propos de 170 cas

Objectif. Présenter et évaluer les limites de la thyroïdectomie trans-orale par voie endoscopique vestibulaire antérieure (TOETVA).

Méthodes. De février 2018 à avril 2022, cette technique a été proposée à des patients âgés de 15 à 70 ans, intéressés par l'absence de cicatrice. Les données pré- et peropératoires, les complications ont été évaluées.

Résultats. 170 patients (164 femmes) âgées de 46 ± 12,4 ans (15-70) dont 13 avec un IMC > 25 et 4 > 35. L'indication correspondait : 20 cancers papillaires, 10 nodules oncocytaire, 24 nodules toxiques, 22 maladies de Basedow et 94 goitres et/ou nodules symptomatiques. Le diamètre préopératoire des nodules était de 38,1 ± 19,9 mm (4-90 mm). Les interventions étaient 101 lobo-isthmectomies, 61 thyroïdectomies totales et 10 isthmectomies. La durée opératoire était de 134 ± 45 min. 2 patients ont été totalisée par une seconde TOETVA et 2 présentaient des cicatrices cervicales d'autre chirurgie. 15 patients ont été convertis en cervicotomie (8,7 %) 8 pour un de nodule > 6 cm, 7 une hémorragie. Les complications étaient 10 parésies récurrentielles transitoires (6,4 %) et 3 définitives (1,7 %), 10 hypoparathyroïdies transitoires (16,3 %) et 2 définitives (3,2 %) bien tolérées. 30 patients ont eu des troubles neuropathiques mentonniers transitoires. Aucun hématome, ni infection ni neuropathie axillaire n'ont été observés. Tous les patients ont déclaré être satisfaits.

Conclusion. La TOEVA est une technique efficace et sans cicatrice pour des thyroïdectomies totales ou partielle dont la taille des nodules est inférieure à 6 cm. La maladie de Basedow, l'IMC ou une cicatrice cervicale ne sont pas une contre-indication. Tous les patients ont bénéficié de kinésithérapie postopératoire.



TRANSORAL ENDOSCOPIC THYROIDECTOMY USING A VESTIBULAR APPROACH (TOETVA):

First experience in France with 170 cases

Objectiv. To present and evaluate the limits of a transoral thyroidectomy using an anterior vestibular endoscopy approach (TOETVA).

Methods. From February 2018 to April 2022, this technique was proposed to patients aged 15 to 70 years, interested in the absence of scarring. Pre- and intraoperative data and complications were evaluated.

Result. 170 patients (164 women) aged 46 ± 12.4 years (15-70), 13 of whom had a BMI > 25 and 4 > 35. Surgical indication corresponded to: 20 papillary cancers, 10 oncocytic nodules, 24 toxic nodules, 22 Graves' disease and 94 symptomatic goitres and/or nodules. The preoperative diameter of the nodules was 38.1 ± 19.9 mm (4-90 mm). The procedures were 101 lobectomy and isthmusectomy, 61 total thyroidectomies and 10 isthmusectomies. The operative time was 134 ± 45 min. Two patients underwent a second TOETVA and 2 had cervical scars from other surgery. 15 patients were converted to cervicotomy (8.7%), 8 for a nodule > 6 cm, 7 for bleeding. Complications were recurrent paresi :Ttransient for 10 (6.4%) and definitive for 3 (1.7%), and hypoparathyroïdies transient for 10 (16.3%) and definitive yet well tolerated for 2 (3.2%). Thirty patients had transient chin neuropathy. No haematomas, infections or axillary neuropathy were observed. All patients reported satisfaction.

Conclusion. TOEVA is an effective and scarless technique for total or partial thyroidectomies with nodule sizes of less than 6 cm. Graves' disease, BMI or a cervical scar are not contraindications. All patients received postoperative physiotherapy.
